How to use your patient portal

Your care team sets your account up with you at your visit. This page walks you through signing in and what to do each week so your provider can see your progress.

Signing in for the first time
Your care team creates your account — you only choose your password.
  1. 1Open the email from Wellness & Balance and tap the confirm link. Your portal switches on as soon as your email is confirmed.
  2. 2Go to wellnessandbalancellc.company/patient-login on your phone or computer.
  3. 3Choose a password you will remember, then sign in.
  4. 4Forgot it later? Use “Forgot password” on the same page and we email you a reset link.

You never need a PIN. If a page asks you for a Hub PIN, you are on the staff sign-in page — go back to wellnessandbalancellc.company/patient-login.

What to do each day
Everything below lives in the tabs at the top of your portal.

My food log

Scan the barcode on a packet with your phone camera, or pick the food from the list. Calories, protein, carbs, fat and fibre fill in for you. Watch the “calories left today” number.

Water

Tap to add each glass as you drink it.

Weigh-in

Step on the scale the same morning each week and enter the number. It draws your chart.

Weekly journal

A few lines on how the week went. Your provider reads every entry.

My care plan

Your goals and your 7-day meal plan. It refreshes with your latest weight.

Messages

Message your care team, and see when we have read it.

How to add your food
Four steps. Read it once and you won't need it again.

1. Choose the meal

At the top, pick Breakfast, Lunch, Dinner, Snack or Beverage. This is just where the food gets filed — you can change it next time.

2a. Scan the packet (fastest)

Tap Scan barcode and hold the barcode on the packet in front of your phone camera. The name, calories, protein, carbs, fat and fibre fill in for you. Say yes if your phone asks to use the camera.

2b. Or pick it from the list

Type the food, brand or restaurant in the search box — for example “Premier Protein” or “Chick-fil-A”. You can also tap Protein drinks or Eating out to show only those. Then tap the food you ate.

2c. Or type it yourself

Can't find it? Use the “Something else” box: type what you ate and roughly how many calories. An estimate is fine — logging something beats logging nothing.

3. Set how many servings

1 is one serving of the size shown next to the food. Half a portion is 0.5, two portions is 2. The numbers double or halve for you.

4. Tap Add

The entry appears in today's list and your “calories left today” number updates. Tap the X next to an entry to remove it if you tapped the wrong thing.

Water

  • Water has its own row — tap 8, 12, 16 or 24 oz each time you finish a glass or bottle.
  • Protein shakes and other drinks you log as food also count toward your water for the day.

Getting it right

  • Log as you eat, not at bedtime — it takes about ten seconds and you won't forget anything.
  • Aim for at least 5 days a week. Your provider looks at this before your visit.
  • Going over your calories is not a failure — log it anyway, it's what we talk about together.
  • The calorie, protein and water goals you see come from the care plan your provider signed. If they look wrong, message your care team.
What to eat when you have no appetite
Eating too little costs muscle, not just fat. Here is what to reach for on the days food does not appeal.

Why eating still matters

  • When you eat far too little, your body breaks down muscle for fuel — not just fat. Muscle is what keeps you strong, steady on your feet and able to lift, walk and climb stairs.
  • Losing muscle also slows the rate you burn calories, so weight loss stalls and comes back more easily later.
  • Protein at every meal, plus movement, is what protects muscle while the fat comes off.
  • A low appetite is not a reason to skip a meal. It is a reason to make the meal smaller, softer and higher in protein.

Sip it (easiest on a poor appetite)

  • Premier Protein or Fairlife Core Power shake — about 30 g protein, counts toward your water too
  • Greek yogurt smoothie with milk and half a banana
  • Broth-based soup with shredded chicken or lentils
  • Milk or a fortified soy milk with a scoop of protein powder

Soft and easy to chew

  • Scrambled eggs or an egg and cheese bite
  • Cottage cheese with peaches or pineapple
  • Greek yogurt with a spoon of peanut butter
  • Tuna or egg salad on a cracker
  • Refried beans, hummus, or mashed sweet potato with butter
  • Baked fish or ground turkey with mashed potato

Grab and go

  • String cheese or a cheese stick
  • Hard-boiled eggs
  • Beef or turkey jerky
  • Protein bar with at least 15 g protein
  • Handful of nuts or roasted chickpeas
  • Rotisserie chicken pulled off the bone

How to get it down

  • Eat by the clock, not by hunger — something small every 3 to 4 hours.
  • Protein first on the plate, before bread, rice or potato.
  • Aim for roughly 20 to 30 g of protein at each sitting; a shake counts.
  • Keep portions small. Half a sandwich you finish beats a full plate you push away.
  • Drink between meals, not during — fluids fill you up fast.
  • Cold or room-temperature foods often smell less and go down more easily.
  • Log whatever you managed. A part-finished meal is still information for your care team.

Tell your care team if

  • You have gone two days or more barely eating anything.
  • You cannot keep fluids down, or you are vomiting.
  • You feel weak, dizzy, shaky or unsteady on your feet.
  • You are losing weight much faster than the plan your provider signed.
  • Your clothes feel looser but you feel weaker rather than stronger.

What we ask of you
This is what your provider reviews before each visit.
  • Log your food on at least 5 days a week.
  • Weigh in once a week — same day, same time.
  • Write one journal entry a week.
  • Open your plan whenever we share a new one.

If you feel unwell, have chest pain, or are in crisis, do not use the portal — call 911, or call or text 988 for the Suicide & Crisis Lifeline.
